Summary & Responsibilities

An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness.

The rising team leader is an individual preparing to take on the role of a team leader. A rising team leader is expected to serve on the ACT team and report directly to the Team Leader.


Job Duties include:

  • Assist Team Lead in the overall supervision of team members as it relates to the delivery of best and ethical practices
  • Participate in rotating on call
  • Work alongside Team Lead throughout the intake process to develop and coordinate initial assessment and initial treatment plan, assign the most appropriate staff to the individual treatment team and provide clinical supervision of the development of the comprehensive assessment and the treatment plan for each client
  • Continuously evaluate the status of clients and do appropriate planning and coordination of treatment activities to ensure immediate attention to changing needs
  • Directly provide ACT services to an individual, where a therapeutic relationship is developed between the individual and the Team Leader


Education and Experience Required: 

  • Bachelor’s degree in Special Education, Psychology, Social Work or closely related Human Services area
  • 2 years of professional work experience in adult mental health
  • Exceptional written, oral, interpersonal, and organizational skills. High level proficiency in Microsoft Office- MS Word, Excel, Access, and PowerPoint.


Education and Experience Preferred:

  • Prefer a master’s degree in a related field.
  • At least 1 years experience in clinical services, primary and behavioral health care integration, organizational change processes, and consultation and training development/implementation.
