Life Transitions is a palliative medicine practice that offers support for chronic illnesses. We do this by working with a person’s doctor to add another layer of expert care to existing treatments. Our goals are to provide better quality of life through symptom management, to offer assistance in making important medical decisions, and to decrease trips to the emergency room and cut down on hospitalizations. Our Newton clinic is open on Tuesdays and Wednesdays from 9am –2pm.
